2.Application guide

If you wish to apply, please prepare for the application by checking the following documents.

Also please obtain a copy of the latest Application Guide from the Graduate Student Affairs Section. It is available at our window. Or you can obtain it via postal mail too. For details, please see " 5.Obtaining a copy of the Application Guide ".

• Please check details about the entrance exam in "Examinee Notes" that will be sent after application.

5.Obtaining a copy of the Application Guide

Obtaining a copy of the application guide from the graduate student affairs section counter, location etc..

Reception hours: 9:00 a.m. to 5:00 p.m. on weekdays

Other important information

Please tell the staffs which Application Guide you require, i.e. Master's Program, Professional Degree Program, Doctoral Program, or Doctoral Program in Medicine and Desired Department.

Requesting a copy of the Application Guide by post

Mailing of the Application Guide may be delayed if you fail to indicate the program for which you wish to sit the entrance exam (particularly for the Doctoral Program and the Doctoral Program in Medicine) and your desired department. Therefore, please confirm your desired program and major before lodging your request.

Request form etc.

The Application Guide can only be obtained either directly from the Graduate Student Affairs Section counter or by postal mail (phone or fax orders will not be accepted). To request a copy of the Application Guide by post, fill out request form (1) below and send it to the below-mentioned address together with a self-addressed envelope (2). Be sure to write '大学院募集要項請求' ('Application Guide Request') in red ink on the front of the envelope.

• Self-addressed return envelope Fill in your name and address, and attach the required postage to the envelope.

※Guide to postage fees Copies Postage Envelope size 1 ¥250 No. 2 Rectangular (240x332mm) If you require express delivery, add the express delivery fee to the postage fee listed on the left. Also, please write '速達' on the reply envelope with red ink. If your sending address is outside Japan, please enclose sufficient IRC(International Reply Coupons, 1 piece of IRC=130 yen)with the request form. We will send application form by EMS(Express Mail Service). If IRC is not available in your region, please enclose International Postal Money Order. Our weight of the Application Guide is about 200grams. The cost depends on the area where you receive. Please confirm here(the rate from Japan post) before sending it. When you enclose IRC or International Postal Money Order, please also include the EMS dedicated envelope or an envelope larger than 240×332mm, 9.45×13.07 inch. In this case, you do not need to fill in your name and address on the envelope, but please make sure to write your address on the "Application Guide Request" form. Please be aware that we will not be able to send the Application Guide if the amount of stamp (or IRC) is insufficient for reply.

You must submit your university/postgraduate transcripts etc. when filing your application so be sure to request the Application Guide well in advance of the application deadline to allow sufficient time to collect all necessary documents.

9.Application for Disclosure of Entrance Exam Information

Application for Disclosure of Entrance Exam Information is accepted during the following period by submitting the format below.

Doctoral Program[October 2023 admission] ：2023/7/27（Thu）～2023/8/3（Thu）

Master's & Professional Degree Program ：2023/8/28（Mon）～2023/9/4（Mon）

Doctoal Program in Medicine ：2023/10/23/（Mon）～2023/10/30/（Mon）

Doctoral Program[April 2024 admission] ：2024/1/31/（Wed）～2024/2/7/（Wed）

Application Submission Office ：1st Floor, Faculty of Medicine Bldg.2.　Graduate Students Affairs Section（Weekdays 9:00～17：00） *Disclosure documents will be sent after the Announcement of Results.（Those who pass the entrance exam will receive it with admission certificate.） *Graduate School of Medicine will accapt the application at counter only. If you are applicable to on of the following, your application will be dealed with by the office of disclosure.

• If more than 2 years have passed since the entrance examination was conducted.
