Applicants are to present

  • the original copy of the NIN slip issued to them after the first registration
  • the printout of the Remita Retrieval Reference (RRR) Number given at the bank after payment/ or a printout stating the payment details if the payment was made online
  • an Application Letter duly signed by the applicant for change in the data fields he/she wishes to modify stating the reason
  • the supporting document to back the proposed changes to be submitted accordingly.

Process for Modification (Adults):

  • Applicant walks into the enrolment Centre with a supporting document (including application letter).
  • Applicant will be issued a data modification form to fill.
  • Applicant data modification forms will be vetted by a NIMC Official to crosscheck the information filled on the form against the supporting document provided. Note: applicants who cannot read nor write should come along with a person who can do the form filling for him.
  • Applicant’s pre-filled information will be inputted on the NIMC Software by a NIMC enrolment officer.
  • The enrolment officer will ask the applicant to double check his/her information using the applicant monitor to check for errors.
  • If the applicant is satisfied with his/her demographic information, the biometrics of the applicant will be captured subsequently.
  • After completing the enrolment, a Transaction ID Slip will be issued to the applicant as an evidence of the transaction.
  • The former NIN Slip will be retained by the enrolment officer if the modified data will affect details on the NIN slip.
  • The applicant will be requested to come back for the NIN within 2-5 working days or more, as it is subjected to speed of the network.
  • A new National Identification Number Slip having the same NIN will be issued to the applicant after processing.

If the modification affects the information on the applicant’s card , the following process will take place:

  • A card replacement form will be filled and the old National ID card retrieved.

The new/modified National e-ID Card will be issued to applicant within three (3) months after re-issuance of NIN slip.

Service Fee: N 500 per field (except for Change of Date of Birth which is N 15,000 for processing your request only – non-refundable) . Payable through the NIMC into the TSA (CBN).

Availability: Monday – Friday (8a.m. to 4 p.m.)

  • Payment must be made before modification commences.
  • Availability of network.
  • Availability of NIMS Backend.
  • Availability of power.

Payment Process

Step 1: Visit www.remita.net to start the payment process.

Step 2: Click on the option ‘ Pay Billers ‘ on the ‘ Bills & Purchases ‘ button.

Step 3: Who do You want to Pay – type in ‘ National Identity Management Commission ‘. Name of Service/Purpose – choose ‘ Modification of any other field other than DOB ‘ or ‘ Correction of Date of Birth ‘. Amount to Pay – will be set based on your choice in the step above. Payer’s Name – type your full name. Payer’s Phone – type your phone number. Tick the ‘I am not a robot’ reCAPTCHA checkbox and click on the stated selection of images before clicking on the ‘Verify’ button.

Step 4: Click on the ‘Submit’ icon to Ížproceed to payment. A new page which is the invoice will come up which will display your RRR – Remita Retrieval Reference at the top of the page. Print the invoice and take note of your RRR.

Step 5: Choose any payment method convenient for you. Option 1: ‘Card’. Option 2: ‘Bank Branch’. Option 3: ‘USSD’. Option 4: ‘Internet Banking’ Option 5: ‘Wallet’. Proceed to complete your transaction by clicking on the “Pay…’, ‘Print Invoice’ or ‘Continue’ button as suited to your preferred method.

Changing the date of birth on a birth certificate is no easy task. Once the original birth certificate has been submitted, it is presumed to be correct. Therefore, it's up to you to prove the date is wrong. If you gather acceptable documents proving the correct date of birth, however, you'll be able to get it done.

Proving the Date of Birth

Step 1 Talk with the hospital.

  • In addition to copies of the original application for a birth certificate, the hospital should have records of the birth itself as a medical event. If the date on the application is incorrect, the medical records of the birth are the most credible document proving the date of birth.

Step 2 Ask your religious institution for baptismal or naming records.

  • Baptismal records are better than no records, to be sure, but they aren’t perfect. In Christian traditions, there is no set time limit dictating when an infant must be baptized. It may take place a few days after birth or a few weeks after birth. Indeed, some churches don’t practice infant baptism at all.

We cannot accept your photo.

Bad photos are the number one reason why we put passport applications on hold. Follow these steps to respond to the letter asking for a new photo:

  • Make sure to include a copy of the letter you received from us, so that we can match your new photo to your pending application.
  • Respond by the deadline on the letter.
  • Don’t write anything on the front or back of the photo.
  • Review our  photo requirements  before sending us a new photo.

You did not appear in person to apply for your child’s passport.

If only one (or neither) parent or guardian applied in person for the child’s passport, you will need to provide more information:

Important : Submit  Form DS-3053  and other notarized statements within three months of signing them.

The document(s) you submitted with your child’s application does not show that you are the parent or legal guardian.

To show parental relationship with your child, submit one of these documents:

  • U.S. birth certificate
  • Foreign birth certificate
  • Adoption decree
  • Divorce or custody decree

If your name has changed since your child’s birth, submit one document showing your name change:

  • Court-ordered name change
  • Marriage certificate
  • Divorce decree

You need to submit a new U.S. birth certificate.

You must submit both the original (or certified) birth certificate and a photocopy. Your U.S. birth certificate must meet these requirements:

  • Issued by the city, county, or state of birth
  • Lists applicant’s full name, date of birth, and place of birth
  • Lists parent(s)’ full names
  • Has the signature of the city, county, or state registrar
  • Has the date filed with registrar's office (must be within one year of birth)
  • Has the seal or stamp of the city, county, or state which issued it

Contact the  vital records office  in the state or territory where you were born if you need a new birth certificate.

If there is no birth certificate on file in the state or territory where you were born, you must submit a Letter of No Record and early public records. See our  Citizenship Evidence  page for more information.

Sample U.S. Birth Certificate

You mailed your Form DS-82 to renew your passport, but something was missing.

When you renew by mail, an intake facility reviews your application and supporting documents before processing starts at a passport agency or center.

If you did not complete the application or provide all supporting documents, the intake facility will send you a letter. The letter may include a QR code with a link to our Renew by Mail page . Common reasons for getting this letter include:

  • No signature or date on your form
  • Bad or missing photo
  • Pages of your form are missing
  • Wrong or missing fees
  • Most recent passport is missing
  • Most recent passport issued over 15 years ago
  • You are not eligible to renew and must apply in person

Follow the instructions in the letter. You must include the letter we sent you when you send the new application.

"Not Available" means that your application is being reviewed by our intake facility and has not arrived at a passport agency or center.  

"In Process" means a passport agency or center is processing your new application. 

You need to give us a new photo ID

When you applied in person, you had to show the acceptance agent a photo ID. If you received a letter or email asking for more identification, send us several photo IDs and include photocopies of the front and back of each one. 

You became a U.S. citizen when your parents were naturalized and need to provide more documents to show it.

If you were born outside the United States and got U.S. citizenship through the naturalization of your parent(s), submit:

  • Your foreign birth certificate listing your parent(s)
  • Evidence of your parent’s U.S. citizenship such as a U.S. birth certificate, Consular Report of Birth Abroad (CRBA), or naturalization certificate
  • Permanent Resident Card/Green Card
  • Foreign passport with the original I-551 visa entry stamp
  • Your parents' marriage certificate (if your parents were married when you legally entered the U.S. and before your 18th birthday) 
  • Documentation of legal custody when you entered the United States, if your parents were not married at that time. If your parents divorced after you entered the United States, provide documentation of legal custody at the time of your parent’s naturalization 
  • Evidence that you resided in the United States in the legal and physical custody of your U.S. citizen parent 
  • Evidence of your legitimation (if your parents were not married at the time of your birth). Legitimation means a father – whose child was born when he was not married – establishes a full legal relationship to his child. Establishing this relationship gives the father the same rights and obligations as if his child had been born while married to the child’s mother.
  • Your parents' marriage certificate dated after your birth
  • Certified court order of legitimation

An Affidavit of Birth is a document submitted to a Court, University, or other institution in order to verify or correct birth information . A person might need to verify their birth information to apply to school or a job or to get a passport, visa, or other government documents. Common situations where an individual might use an Affidavit of Birth include:

  • A birth record that has been misplaced or destroyed
  • An incorrect or incomplete birth record
  • Supplementary proof of birth information submitted along with a birth certificate

Once an Affidavit has been completed, it can be used as a convenient alternative to an official birth certificate .

How to use this document

This document should be completed by a person, known as the Affiant, who can attest under oath to their personal knowledge of the circumstances surrounding the subject's birth . This person could be a relative, like a parent or a grandparent, or someone who was present at the birth, like a physician or close family friend. The document includes information identifying the Affiant, including their name, address, and birth date. The document then goes on to list information identifying the subject of the Affidavit, like their name, gender, age, and citizenship. Finally, the document explains how the Affiant and the subject know each other and the source of the Affiant's personal knowledge about the circumstances surrounding the subject's birth.

Once the Affiant completes the Affidavit, they should sign it and have it notarized . The subject of the Affidavit should keep the notarized Affidavit in a safe place, such as safety deposit box, so that it is always available to use when they need proof of their birth.

Applicable law

There are no laws outlining exactly what must be put into an Affidavit of Birth. There are, however, some generally accepted practices for creating such documents, including making sure the information is robust enough to be persuasive to an authority that the Affiant is qualified to attest to the circumstances surrounding subject's birth.

Handling changes of date of birth information on citizenship clients (documentation and GCMS)

This section contains policy, procedures and guidance used by IRCC staff. It is posted on the department’s website as a courtesy to stakeholders.

How the decision to change date of birth is made

The decision regarding a change to a date of birth will usually be made at the beginning of the process. This means that in most cases, the decision to change the date of birth for citizenship grant or proof applications rests with the Program Support Unit in the Case Processing Centre in Sydney (CPC-Sydney). It is an internal policy at CPC-Sydney to have two officers review a file and documents when a decision may result in a denial of services or a negative decision. The decision, however, rests with one officer.

When a case is referred to Case Management Branch (CMB), the case review officer makes the decision. Once a decision has been made regarding the date of birth, the decision will not be revisited by another officer in CPC, CMB or in the field unless there is evidence to show that the decision-maker was missing relevant information to render the initial decision.

If request to change date of birth is received mid-process

An application is considered in mid-process up until the time the oath is taken. As the objective of the policy is to ensure that citizenship records contain accurate information, citizenship applicants may request a different date of birth after submitting an application. However, when a client asks for a change to the date of birth in mid-process, the client will be advised that the application will be returned to CPC-Sydney for assessment and decision AFTER the client has submitted the information from Operations Support Centre (OSC) and the completed questionnaire (CIT 0464) with documents, if applicable.

Advise the client that regardless of whether the new date of birth is approved by OSC or for citizenship purposes, the citizenship application process will be delayed. In all cases where a client submits information about a different date of birth, new clearances will be required even where the client decides not to go forward with a change in the date of birth or where the request is made but the decision is not to change the date of birth.

Corrections to the certificate will be allowed after the oath was taken where there was an administrative error by the Department such as a typographical error.

Making a negative decision

When the decision is made to refuse a request for a change to the date of birth, the officer who makes the decision will write a letter to the applicant. The letter will explain why the request is refused and what the client’s options are.

Clients may provide further information or evidence to the decision-maker and continue with attempts to change the date of birth, or clients may drop the request to change the date of birth and simply proceed with the application using the date of birth on the IMM 1000, Confirmation of PR, PR Card or, if applicable (proof applications), in the original citizenship records.

When to approve a request for a change to the date of birth

There are some situations where it is reasonable to expect the decision-maker to approve a request to change a date of birth. The following are examples of such situations:

The applicant does not have to submit an IMM 1436, nor does the client have to provide supplementary documents. In these cases, the CPC will inform OSC of the change so that OSC can update their records by entering an NCB in FOSS. The information collected through the questionnaire (CIT 0464) will assist OSC in determining whether an investigation is necessary.

  • Administrative error: (if CPC-Sydney issued the certificate with a typographical error). The error may have been as a result of incorrect data entry or because the client transcribed the date of birth incorrectly on the application form and the error was not caught during the application process. For example: Applicant’s date of birth per the IMM 1000 or Confirmation of PR is October 12, 1948. The client indicated 1948-12-10 on the application form and this was interpreted as December 10, 1948 on the certificate.
  • Date of birth as per IMM 1000, Confirmation of PR, or PR Card: The certificate was issued using a date of birth on a passport or other document and the subject applies to have a certificate with the date of birth as per the IMM 1000, Confirmation of PR or PR Card. For example: Applicant applied for grant of citizenship in 1994 and initially requested a date of birth as per the IMM 1000. The certificate was issued using the birth date on the passport, which was different. The applicant requests a replacement certificate with the date of birth that appears on the IMM 1000.

Factors to consider

Information and documentation.

There is no one document or list of documents an applicant may use to substantiate the birth date change. In most cases, it is reasonable to expect the applicant will submit primary documents, including an original birth certificate or amended birth record from their country of birth. In other cases, the evidence submitted will consist of secondary documents such as birth or death notices, affidavits, entries in family records or census records. The decision-maker must review all the evidence submitted. The evidence includes documents and the completed questionnaire and, if applicable, the written decision from the OSC for why they refused the Request to Amend Immigration Record of Landing (IMM 1000) or Confirmation of Permanent Residence (IMM 5292). Some of the factors the officer should consider when reviewing documents include:

  • When were the documents issued? Were they issued before the date of landing? Before the acquisition of citizenship?
  • From where are the documents issued? Is it a country known for providing reliable documents or are most documents from this country considered to be unreliable?
  • Do the documents meet the standards for that country?
  • Are the answers to the questions in the Request to Correct a Date of Birth for Citizenship (CIT 0464) consistent with the evidence submitted?

Identity and misrepresentation

A change to a person’s biographical data can sometimes result in a change to the person’s identity. Changes to biographical data could also result in a person gaining an advantage to which they are not otherwise entitled. For example, a person may be changing the birth date in order to access pension benefits sooner than the person would normally be eligible. While the citizenship certificate is to reflect the true identity, the officer making the decision about a birth date change needs to be aware of the consequences of changing a person’s identity or biographical data. The following are factors to consider in order to determine whether there is an identity issue or whether the client may be misrepresenting the information for gain.

  • Is this applicant requesting a minor change or a major change to a date of birth?
  • Is this request accompanied by a request to change something else such as a person’s name or place of birth?
  • Did the applicant initially present a “primary” document from one country and is now presenting another “primary” document from a different country?
  • Is the applicant providing a document that they previously claimed was impossible to obtain? How did the applicant obtain this document?
  • Does the change affect the person’s initial category of immigration to Canada? Does it affect the eligibility for sponsorship (i.e., age for sponsorship)? These cases should be referred to OSC for possible immigration investigation.
  • Will the applicant receive benefits as a result of the change (e.g., is suddenly eligible for pension)?

Where the officer reviewing the file has any doubts regarding a person’s identity or where the officer suspects there may be misrepresentation, the officer will refer the case with information to CMB. In some cases, CMB will refer the case to OSC for possible immigration investigation where it appears the information may have affected the person’s initial right to enter Canada or initial right to sponsor or be sponsored.

Other considerations

The following list is not exhaustive but serves to provide the officer with some factors to consider when making a decision about a request to change a birth date. The factors are reflected in the Request to Correct a Date of Birth for Citizenship (CIT 0464) questionnaire.

  • Timing of application: Why is the client requesting to change the date of birth at this time? When was the error discovered? If the client discovered the error many years ago, why are they requesting a change at this time?
  • Date of issuance of documents submitted to support new date of birth: Can the client provide documents which predate the date the error was recognized? Are the documents issued as a result of the client recognizing that there was an error?
  • Representation made to other organizations: Has the client tried to change the date of birth with other Canadian organizations (e.g., driver’s licence, health care, pension benefits, school records)?
  • Decision made by OSC: What was the reason for the refusal by OSC? The information provided to OSC may be used in making a determination on the change of the date of birth.
  • Previous steps taken to modify the date of birth: What attempts have been made to modify the date of birth? Has the client made continuous attempts to modify the date of birth since the error was detected?
  • Motivation of the person: Are there benefits derived from the change of date of birth?
  • Number of requests made to modify the date of birth: Is this the first time the client has tried to modify his date of birth? Has the date of birth been previously changed, such as with an adjustment certificate?
  • Reliability/Validity of documents: Are there questions regarding the validity of the documents?
  • Minor change vs. major change: Is the applicant requesting a change of a few days or many years?

The pass port office nowadays insisting persons born on or after 1989 applying for passport to have a birth certificate and in every state, the corporations have computerized the data from the manual records and while on searching on line there are corrections, changes in the data like, name, date of birth, father's name, mother's name. So here is a sample letter to request the corporation authorities to do the needful and issue a fresh certificate.

Hi Shwan You have to approach to state government authorities with your plea along with a copy of birth certificate and school leaving certificate for correction in name. If the date of birth belongs to before October 1989, the effect to the notary public certificate is sufficient to deal for getting other documents like passport etc., If it is after October 1989, it may be difficult to have two different date of births as government has started registration of births mandatory. You may have to consult a lawyer to this effect.

Yes Mohamed Aamir You have to get it only from the place where you have born and for the people who born after 1989, the birth certificate with all its correct data is needed to apply for passport. Previously people managed with notary public affidavit but now it is not so. If you have any one in Mumbai now, you may approach through them otherwise you may have apply in person (though the data are now available in net) and the all the corrections you may have to do at once because authorities may not entertain one by one if applied later. So go through your current certificate these three details, date of birth (also check with your educational qualification certificates that the date is entered correctly), father's name and own name. I have done for my son in Chennai one year back (at his age of 21 for applying to passport) but in Chennai it was really cumbersome.
