I have informed Mr.X regarding the work that has to be done on an urgent basis, and i might be able to respond to calls or emails if my health permits or if my attention is needed on an urgent basis.

I request that you kindly accept my request for sick leave for the aforesaid period.

5. Express Gratitude

If you are informing your manager on short notice, thank your employer for their understanding and offer your apologies. For instance,

  • I appreciate your support
  • I thank you for understanding my situation
  • I apologize for the inconvenience caused on such short notice and thank you for your understanding.

6. Closing with Signature or Name

End the application formally with-

Yours sincerely,

[Your Name]

[Signature]

You can also use other complimentary notes such as “Best Wishes”, “With Regards”, or “Yours Truly.”

7. Attach a Supporting Medical Document

Tips for Writing the Letter

  • Resignation Letter for Health Reasons

Email Resignation for Health Reasons Example

 Maddy Price / The Balance

Are you resigning from a job for medical reasons? When you need to leave your job due to health issues, you may choose to let your employer know the reason for your resignation letter.

How much information you share is entirely personal. Some employees are comfortable discussing personal information with their managers, especially if they have a friendly relationship. Others maintain a professional distance and don’t wish to disclose private circumstances at work. You don't have an obligation to share your reason for resigning with your employer. Below you'll find a sample resignation letter and email you can consider as you write your own.

Check Your Eligibility for Leave Benefits

Before you resign, be aware of your rights and any benefits you may be entitled to during your illness. Do note that, depending on your circumstances, you may qualify for assistance under the Family and Medical Leave Act (FMLA), worker's compensation, or disability benefits, or you may be eligible to  take a leave of absence  rather than resigning.

Confirm whether you are eligible for leave with your manager or human resources department before submitting your resignation.

When you  resign from your job , it’s always a good idea to  speak with your manager in person , when possible. You should strive to provide  two weeks' notice  as well, or the amount of time recommended in your employee handbook or employment contract.

Unfortunately, sometimes circumstances arise that require you to  resign immediately , and, if you need to, you should do so as gracefully as possible.

What to Include in Your Resignation Letter

You are not obligated to provide a reason when you resign. If you prefer to keep your health issues private, it's acceptable to write a  simple note stating you are leaving  and informing your employer of your last day of work.

If you prefer to share some of the details, you can inform your employer you are resigning because of your health and provide as much or as little information as you are comfortable with.

You should try to provide notice two weeks in advance of your final day of work when possible, although you may be unable to give notice if your situation dictates.

Your  resignation letter can be submitted by email  or as a business letter. In an email, your subject should be clear and concise: "Resignation—Firstname Lastname." A business letter should begin with your contact information, followed by the date and your manager’s contact information. Some other tips:

  • Begin your letter with a salutation: Dear Mr./Ms. Lastname, or Dear Firstname, depending on your relationship and the formality of your office.
  • Your first paragraph should include two pieces of information—that you are resigning from your position and your last day of work.
  • If you wish to share your reasons for leaving, you can use your second paragraph to explain.
  • Your final paragraph should express your appreciation for the time you shared with the company and the opportunities you had there.
  • Close with your signature: Sincerely,  Firstname Lastname  followed by your typed signature. In an email, your contact information should be included in your signature.

If submitting your resignation via letter, this is a template you can follow:

Your Name Your Address Your City, State Zip Code Your Phone Number Your Email

Name Title Organization Address City, State Zip Code

Dear Mr./Ms. Lastname:

It greatly saddens me to send you this letter of resignation. Effective the end of this month, I will no longer be working here as a P.E. teacher.

Recently I have been noticing some changes in many aspects of my life, both personally and professionally. I have been overly exhausted, constantly in pain, and feel that my productivity has been cut in half. I went to a doctor, and I have been diagnosed with Fibromyalgia, a condition that involves chronic pain and soreness. Because of the high activity level consistent with my work, I am no longer able to teach my students effectively, and I do not want them to be impacted by this. My doctor agreed with me that this was the best decision for all involved.

I have so enjoyed my time here at FMA Middle School. My work gave me great satisfaction, and I never will forget the last 20 years of memories, incredible friends, and colleagues that I have gained. I hope we will still stay in touch despite my early exit.

Please let me know if there is any way I can help in finding my replacement. Although I no longer can do the work I once could, I hope to remain a resource and that we will stay in touch. Thank you so much for all the opportunities, and I wish everyone at FMA the very best.

If you need to resign via email, you can use this sample as a template.

Subject: Resignation—Firstname Lastname

Dear Mr. Manager,

I regret to inform you of my resignation, effective May 12, 2024. Due to a recent diagnosis, I have become aware that my illness will require extended treatment and recovery, and I am unsure my ability to perform the duties of my present position will return.

I appreciate your understanding. If there is anything I can do to help during the transition, please let me know.

The best way to start a Healthcare cover letter is by directly addressing the hiring manager, if their name is known. If not, "Dear Hiring Manager" is appropriate. Begin by introducing yourself and stating the position you're applying for. Then, briefly mention your most relevant qualifications or experiences that make you a strong candidate for the role. It's also beneficial to show enthusiasm for the healthcare field and the specific organization you're applying to. This shows you've done your research and are genuinely interested in the role.

Healthcare professionals should end a cover letter by summarizing their interest in the position and the unique skills they bring. It's important to express enthusiasm for the opportunity and to thank the reader for their time. For example, "I am excited about the opportunity to contribute my skills and experiences to your team and am confident that I can bring value to your organization. Thank you for considering my application. I look forward to the possibility of discussing this opportunity further." It's also crucial to include contact information, even if it's already on your resume. This makes it easier for the hiring manager to reach you. Always end with a professional closing like "Sincerely" or "Best regards," followed by your name.

A Healthcare cover letter should ideally be about one page in length. This is because hiring managers often have many applications to go through, so a concise, well-structured letter is more likely to grab their attention. In terms of word count, aim for between 250 to 400 words. This length allows you to clearly and succinctly express your interest in the position, highlight your most relevant skills and experiences, and explain why you would be a good fit for the healthcare organization. Remember, the cover letter is your opportunity to make a strong first impression, so every word should count.

Writing a cover letter with no experience in healthcare can seem daunting, but it's all about showcasing your transferable skills, passion for the field, and eagerness to learn. Here's a step-by-step guide: 1. **Start with a Professional Greeting**: Address the hiring manager by name if possible. If you can't find their name, use a professional greeting like "Dear Hiring Manager". 2. **Introduction**: Begin by stating the position you're applying for. Mention where you found the job posting. If someone referred you, mention their name and position. 3. **Express Your Interest**: Explain why you're interested in the healthcare field. This could be a personal reason or a professional goal. Show enthusiasm and passion for the field. 4. **Highlight Transferable Skills**: Even without direct experience, you likely have skills that are relevant to healthcare. These could be soft skills like communication, empathy, and problem-solving, or hard skills like data analysis or proficiency in a second language. Use specific examples to demonstrate these skills. 5. **Education & Training**: If you have any education or training related to healthcare, be sure to mention it. This could be a degree, a certification, or even a relevant course or seminar. 6. **Show Willingness to Learn**: Emphasize your eagerness to learn and grow in the healthcare field. This shows potential employers that you're committed and motivated. 7. **Conclude Professionally**: Thank the hiring manager for considering your application. Express your hope for the opportunity to further discuss your qualifications. 8. **Sign Off**: End with a professional sign-off like "Sincerely" or "Best regards," followed by your full name and contact information. Remember to keep your cover letter concise and to the point. Use a professional tone and language, and proofread carefully for any errors. Tailor your cover letter to each job application, focusing on the skills and qualifications mentioned in the job posting.

I am writing this letter to submit my request for job transfer based on medical grounds.

As you are aware that I was on leave for 15 days in the last month because of my surgery. I was given a long list of post-surgery precautions to be kept in mind in relation with food and lifestyle practices. But because I am staying away from my family because of my work, I am unable to follow the list of do’s and don’ts given by the doctor because after work, it is not possible for me to focus on these guidelines. And as a result, my recovery after the surgery has been poor.

Therefore, I am writing this letter to kindly consider my request for job transfer to our Gurgaon office where my family puts up. I would be really thankful if you could consider my situation and have me transferred for a limited time so that I can focus on improving my health without having my work suffer.

I will be anxiously waiting for your response and me and my family will be very thankful to you if you accept this request.
